Process to Follow To Register:
Procedure for Dungannon Practices.
Should you wish to register within the Dungannon area, you should attend any practice to request registration. The practice will take your details, name, address, etc and add it to a 'Patient Registration Request' list. This list will be forwarded to the local Health Board for allocation to a practice in the Dungannon area. The practice you have been allocated to will then contact you to advise you to attend for registration.
When attending the practice you have been allocated to, you should attend with your medical card. If you do not have a medical card or you have not been registered with a doctor in the UK previously, you will be given a form called a HS200 or a HS22X to complete. This form is also available to download via this site.
PROOF OF IDENTIDY:
All patients registering with the practice must provide proof of identidy. We require 2 forms of identidy - (i) Photographic ID (ii) Proof of address. Unfortunately we will be unable to process your registration until we are in full receipt of the information requested. Please ensure you bring this information with you when leaving in your registration form.
All patients must attend the Practice Nurse for a pre-registration health check. An appointment will be scheduled for you. Please contact reception for further information, a member of our reception team will be happy to answer any questions you may have regarding registration.
We treat all patients within an 8 mile radius of Dungannon.
Anyone staying or working temporarily in the area and in need of urgent medical attention or advice is welcome to make an appointment to see the doctor or nurse. Emergency medication and contraceptive advice can also be provided.
N.B it is not essential to have your medical card with you. Visitors from some countries may have to pay for treatment .
