NEW OPENING HOURS:

Dear Patients

Due to essential administrative requirements the practice has had to adjust our current opening hours. The new hours as follows will take effect from Monday 31st October 2016.

Open: 8.30am - 1pm

Closed: 1pm - 2pm

Open: 2pm - 5.30pm.

The practice will be contactable by phone from 8am - 6pm.

Please note: During the times of 8am - 9am, 1pm - 2pm & 5.30pm - 6pm, the practice will take EMERGENCY CALLS only. (If you ring during these times & your condition is not an emergency, we will politely ask you to contact us back during the times stipulated)

PRESCRIPTION LINE OPENING HOURS

The prescription line will be open during the following hours:

9am - 12pm

2pm - 3.00pm

At present only 18% of patients on repeat medication are signed up online. We would like to encourage patients to sign up to online prescription ordering.  Ordering online is quick and convenient not only to you as a patient but also to our staff processing the prescription.  If you have access to a computer, please consider this. It will free up the dedicated prescription line, leaving it easier for those with no access to a computer to get through. Please see information on how you can do this under 'Prescriptions' heading on our home page.

IMPORTANT NOTICE:

MISSED APPOINTMENTS!!

Please make sure that you attend all booked appointments with the GP.  Missed appointments cause unnecessary delays for other patients who need to see a doctor.  It will only take 2 minutes of your time to cancel your appointment if you don't need it.  This can be done on the telephone or via this website.  Cancelling unwanted appointments will help us provide a better service for all our patients!

New Patient Registration

steth_book1Procedure for Dungannon Practices.

Dungannon practices now register patients on a rota basis. Should you wish to register within the Dungannon area, you should telephone any practice to enquire about this process. The practice on rota will take your details and advise you further on your registration. 

When attending the practice you are registering with, you should attend with your medical card.  If you do not have a medical card or you have not been registered with a doctor in the UK previously, you will be given a form called a HS200 or a HSCR1 to complete. 

PROOF OF IDENTIDY:

All patients registering with the practice must provide proof of identity.  We require 2 forms of identity - (i) Photographic ID (ii) Proof of address. Unfortunately we will be unable to process your registration until we are in full receipt of the information requested. Please ensure you bring this information with you when leaving in your registration form.

All patients must attend the Practice Nurse/HCA for a  new patient registration health check.  An appointment will be scheduled for you.  Please contact reception for further information, a member of our reception team will be happy to answer any questions you may have regarding registration. 

Practice Area

We treat all patients within an 6 mile radius of Dungannon. 

Temporary Residents 

Anyone staying or working temporarily in the area and in need of urgent medical attention or advice is welcome to make an appointment to see the doctor or nurse.  Emergency medication and contraceptive advice can also be provided. 

N.B it is not essential to have your medical card with you.  Visitors from some countries may have to pay for treatment .
